This GROUP Volunteer Application is used when volunteers are committing to a one-time service at the Mission. The Lead Volunteer will complete this form listing all volunteers participating. Please return this completed form to the Volunteer Coordinator either via email or bring to the Mission on the day of service.

Note to Lead Volunteer: 1) Please complete the Individual Volunteer Application and return to the Volunteer Coordinator prior to service 2) Day of service, please have each volunteer sign the "Sign In / Sign Out" log at the Mission.
